We know, Itachi is already one of the strongest and smartest characters in the Naruto series, with just a Mangekyo Sharingan and an Imperfect Susano'o.
First, lets start with the Eternal Mangekyou Sharingan...
If Itachi was to get the Eternal Mangekyou Sharingan, he would have to first take Sasuke's sharingan (I know he would never do that, but lets just say). Now with doing that Itachi won't have to worry about losing his eye sight and it will allow him to use his Tsukuyomi and Ametarasu without much defect. He will also be able to perform Susano'o with more ease and more repeatedly, as well as his Susano'o would get some upgrades, (This makes him super OP, considering he also has the Totsuka Blade and the Yata Mirror).
Basically the Eternal Mangekyou Sharingan only gets rid of the negative impact form the Mangekyou Sharingan and adds in a upgraded Susano'o.
With this sort of power, it seems Itachi would fall a bit short of Madara in the Eternal Mangekyou Sharingan state. Who with that power fought off a Sage Mode Hashirama. Though this I think would definetely make Itachi stronger than Eternal Mangekyou Sharingan Sasuke. Eternal Mangekyou Sharingan Sasuke didn't show off any superiority when compared to Mangekyou Sharingan Itachi during thier battle with Kabuto (Granted Itachi was Edo).
Now lets say Itachi also got his hands on some Senju DNA. With the Eternal Mangekyou Sharingan and Senju DNA, Itachi would be able to awaken the strongest occular power in Naruto, the Rinnengan. With Rinnengan he would the powers of all 7 paths of pain, as well, if he uses the Senju DNA as effectively as Madara, will be able to regenerate. Knowing Itachi, his usage of the Rinnengan will far surpass Madara, as Madara was never seen using most of the abilities the Rinnengan has to offer, however Itachi is a humble guy who uses everything and anything available to him. To top it all of, he would get a perfect Susano'o. A giant Susano'o, with some capable of flight and some capable of effortlessly slicing mountains. Though it is impossible to predict what his would be like, I would like to think his would be able to fly. One because Sasuke's could and thier brothers, and second because Sasuke also had Itachi's eyes, to create EMS.
Going back to the usage of the Rinnegan, Itachi like many others would also cast out a variety of Chakra nature transformation jutsu, which niether Madara, Sasuke nor Obito are ever shown doing, though do have the ability to, with thier Rinnegan. This is really due to Itachi's humble and resourcful character, which is different from most Uchiha who are known to be arrogant and picky about what Jutsu they use.
I personally think, that a Rinnegan Itachi could faceoff against a Rinnegan Madara, in a too close to call battle. But I do have to lean a little bit towards Madara, because of his destructive force and the fact that I believe Sasuke's perfect Susano'o is weaker than Madara's.
That's all I have to say to this matter. Itachi had a lot more potential, but due to circ*mstances didn't get to utilize it all.

Itachi could barely handle using Susanoo for more than a minute in his prime, the abilities of the Rinnegan are even more chakra-extensive. The only advantage he'd gain from this would be that he wouldn't have become blind, which ultimately, is irrelevant because he wasn't going to live to see the war anyway.
Itachi already possessed great power without Shisui's eye, the benefits of implanting it wouldn't override the effects that the respiratory illness had on him. So it would be wasted potential.
So from the two EMS we've seen, it seems that the pattern from the eye donor is added to the eye but it adapts to the original pattern. For example, Itachi had a curved three-bladed shuriken shape while Sasuke had a straight atom shape. Itachi's pattern was adapted to Sasuke's and straightened out.
Itachi didn't get the eternal mangekyou because he just didn't want to. Eternal Mangekyou only requires spare eyes. However, to obtain the Mangekyou you need to be attached to someone, like a sibling/close friend (this explains why Obito never lost his vision and why Kakashi was able to use the perfect sussano).
Could Itachi defeat Madara if he had the EMS? A very slime chance. While he doesn't damage his eyes any longer, he still doesn't have the chakra or stamina to last. Madara only loses if he gets overconfident and attempts to tank a stab from the blade of totsuka.
Sasuke has much more chakra, but Itachi is more intelligent. Sasuke's faster than Itachi but both their sharingan can read movement. Sasuke's Susanoo sword is just as much an instant kill due to the Amaterasu flames the sword can produce, but Itachi has the Yata mirror which gives it better defense.
Because the transplanted eye can use Kotoamatsukami only once every ten years, its premature use on Itachi has ruined his original plan. As such, Itachi destroys the crow with Amaterasu so that Shisui's eye won't fall into the wrong hands.
No. When Sasuke implanted Itachi's MS, Sasuke's brain chakra which strengths his sharingan became more powerful and converted the eye into an EMS but it is still Sasuke's brain chakra and therefore the same powers.

Logically speaking no. None of his genjutsu's would work on madara or obito. And shisui's eye even if he bad both wouldn't help. But with that said, I genuinely feel as if Itachi would beat Madara, not because of logic but because clearly Kishimoto never wanted Itachi to loose a fight.
So, in short, Kakashi cannot activate Mangekyou Sharingan because he doesn't have Uchiha's blood. However, Obito can act as his trigger to activate Mangekyou Sharingan for both of them.

Sasuke seemed to be relatively un-phased. Furthermore, later on, with his EMS Perception he was able to keep track of Juubito at all times, while KSM Naruto had to base his attacks on predictions. Considering all this, it becomes apparent that EMS Sasuke will destroy nearly every Path of Pain in close combat.
Madara Uchiha was the first Uchiha to awaken the Mangekyō Sharingan. It's not known what the individual abilities of each of his eyes were, but he was able to use Susanoo.
Well Naruto is already essentially a demigod, possessing Senjutsu Six Paths + Chakra of all Bijuu. If you add EMS and Rinnegan Sasuke to this power, then Naruto's power is already It will be at level of "God" in the world of shinobi.
With rinegan u get 7 powers with one or two special powers . Like madara getting limbo , sasuke could have had another ability if he had 2 rinegan s . So rinegan beats ms , uchiha with a rinegan bets ems . There is no such scaling in naruto.
He was able to awaken Rinnegan only after having Hashirama's cells implanted in him, which has the Ashura's chakra. But Itachi did no such thing and he couldn't go beyond the Mangekyo Sharingan.
