Create a form with Microsoft Forms (2024)

With Microsoft Forms, you can create surveys, quizzes, and polls, and easily see results as they come in.

Start a new form

  1. Go toMicrosoft Forms with your Microsoft 365 school credentials, Microsoft 365 work credentials, or Microsoft account (Hotmail, Live, or Outlook.com).

    Note:You can also log in to Office.com, look for the Microsoft 365 icons on the left, and then select Forms.

  2. Select Create a form with Microsoft Forms (1)New Form.

  3. Title your formand, if you want, a description of it.

    Note:Form titles can have up to 90 characters. Descriptions can have up to 1,000 characters.

    Note:Your form is saved automatically while you create it.

Add questions

  1. Select Create a form with Microsoft Forms (2)Add new to add a new question to your form.

  2. Choose what kind of question you want to add, such asChoice, Text, Rating, or Date question types. SelectMore question types Create a form with Microsoft Forms (3) forRanking, Likert, File upload, or Net Promoter Score® question types. To organize sections for your questions, selectSection.

    Tip:You can also format your text. Highlight a word or words in yourtitle or questions, and then choose any of the following:Bold(keyboard shortcut -CTRL/Cmd+B),Italic(keyboard shortcut -CTRL/Cmd+I),Underline(keyboard shortcut -CTRL/Cmd+U),Font color,Font size,Numbering, orBullets.

    Important:If you delete a question, it will be permanently deleted along with any response data that's been collected for it. Learn more.

Preview your form

  1. On a computer, select Create a form with Microsoft Forms (4)Previewto see how your form will look. On a mobile device select Create a form with Microsoft Forms (5)to see how your form will look.

  2. Totest out your form, answer the questions in Preview mode, and then selectSubmit.

  3. To keep editing your form, select Back.

Start a new form

  1. Sign in to Microsoft 365 with your school or work credentials.

  2. Select Create a form with Microsoft Forms (6)New> Forms for Excel.

    Note: Forms for Excel is only available for OneDrive for work or school and new team sites connected with Microsoft 365 Groups. Learn more about Microsoft 365 groups.

  3. Enter a name for your form, and then selectCreate.A new tab for Microsoft Forms will open in your web browser.

    Note:Form titles can have up to 90 characters. Descriptions can have up to 1,000 characters.

Add questions

  1. Select Create a form with Microsoft Forms (7)Add new to add a new question to your form.

  4. A new tab for Microsoft Forms will open in your web browser.

  5. Select the placeholder default title and change with your own. If you want, also add a description.

  4. A Forms for OneNote panel will open and dock on the right side of your OneNote notebook.

  5. Under My Forms, select Create a form with Microsoft Forms (13)New Form.

  6. A new tab for Microsoft Forms will open in your web browser.
