Make sure you have the latest version of the Microsoft Authenticator app installed on your device. Check for any available updates in the app store and install them if necessary.

Clear the cache and data of the Microsoft Authenticator app:
On Android: Go to Settings > Apps > Microsoft Authenticator > Storage > Clear cache/Clear data.

Verify that the correct account is added in the Microsoft Authenticator app:
Open the Microsoft Authenticator app.

If none of the above steps work, you can try removing and re-adding your account in the Microsoft Authenticator app:
Open the Microsoft Authenticator app.
Navigate to the account list or settings section.
Find the account associated with Microsoft Teams.
Remove or delete that account from the app.
Add the account again by scanning a QR code or entering the necessary details.

On your computer, go to the Security info page of your account dashboard. Note: If you don't see the Security info page, go to Additional security verification, then select the checkbox next to Authenticator app, and then select Configure to display your QR code.
You can check under your Microsoft account>Privacy>Sign in Activity>View my activity. It'll give IP address, Device, location & browser used. This should show any attempts.

On your mobile device, open Authenticator, or tap the New sign-in request notification. Note: If you're a work or school account user, you may need to unlock Authenticator before confirming sign-in. Tap or enter the corresponding number, then Approve.
