Dealing with credit card companies can be stressful, especially if you need to get a charge you didn’t acquire from your bill and records with the company. The charge be removed.
Any finance and/or other charges related to the fraudulent charge be credited to my account.
I receive an accurate statement ASAP.
These requests are being made pursuant to the Fair Credit Billing Act’s amendments to the Truth in Lending Act, 15 U.S.C. §§ 1666-1666b, 12 C.F.R. § 226.13. See also 12 C.F.R. § 226.12(b).
I have enclosed copies of [list the documents you have as evidence of the fraud] as evidence that these charges are fraudulent. Please investigate this matter and correct the billing error as soon as possible.

Include copies of your evidence with the letter when sending it.
Once this letter has been sent, the company will typically open up its own investigation into your allegations. Generally, it will also send you a letter regarding its findings once the investigation has been concluded.
Even if you are able to successfully dispute a charge with a credit card company, you may also need to follow up with one or all of the credit reporting bureaus to get your records cleared up with them.
