Under the Fair Credit Reporting Act (FCRA), you are entitled to a free copy of your consumer disclosure report, upon your request, at a minimum once every 12 months and more frequently under certain conditions. To request your free report, follow the instructions provided below to submit online or select the ‘Other Methods’ tab for instructions on how to submit through mail or telephone.
ChexSystems® provides all consumer disclosure reports free of charge. You can request a report to learn what information, if any, is listed in your consumer file at ChexSystems. If you have been denied an account and ChexSystems was used in the decision process, this information will help you understand what may have contributed to that decision.
You must be 18 years of age or older to communicate with ChexSystems. ChexSystems may access, store and use your identifying information to the extent permitted by law.

Submitting a request for a minor
You must be 18 years of age or older to communicate with ChexSystems. To submit a request on behalf of a minor, a parent or legal guardian must complete the Consumer Request for Disclosure Form and return it to ChexSystems by mail.
All
of the following documentation must be included:
A copy of the minor's birth certificate;
A legible copy of the minor's Social Security card;
A legible copy of the parent or legal guardian's driver's license or state identification card;
Proof of address for the parent or legal guardian (in the form of a pay stub, utility bill or other official document bearing the address to which correspondence is to be sent);
If your name does not appear on the birth certificate, a copy of a document confirming legal guardianship is required. The proof of guardianship must be an official court or other legally binding document; and
Correspondence must include consumer’s full name, current address, date of birth, and Social Security number.
Submitting a request for someone who is not a minor
To submit a request on behalf of someone who is not a minor, you must complete the Consumer Request for Disclosure Form and return it to ChexSystems by mail. You must also include the following documentation authorizing ChexSystems to communicate and provide information to you:
A notarized Power of Attorney or
A notarized copy of specific written instructions signed by the consumer
Correspondence must include consumer’s full name, current address, date of birth, and Social Security number.

The automated phone system is available 24 hours per day. Representativesare available to assist during normal business hours 8:00am -7:00pm Central Time, Monday through Friday.
Pursuant to the Fair Credit Reporting Act, you may request a Consumer Disclosure Report online via the Online Request Form. Once we have verified and processed your report request, you will receive a letter via U.S. Mail with additional information explaining how to access your report online.

To request a copy of your report, visit the ChexSystems website. Once the agency receives your request, you can expect to get a copy of your report within five business days.
You can contact the LexisNexis Risk Solutions consumer center at https://consumer.risk.lexisnexis.com/ or by telephone at 866-897-8126 to request a copy of your file.
You are also entitled to a free credit report every 12 months from each of the three nationwide consumer reporting companies—Equifax, TransUnion, and Experian. You can request a copy through AnnualCreditReport.com .
How long do you stay on ChexSystems? ChexSystems retains record of reported information for a period of five years from the report date unless the source of the information requests its removal or ChexSystems becomes obligated to remove it under applicable law or policy.
If you dispute the completeness or accuracy of the information in your consumer file, you have the right to submit a dispute directly to the source of the information. However, if you prefer, ChexSystems® will contact the source on your behalf to initiate a reinvestigation.
The report shows a breakdown of specific accounts closed, outstanding debts, bounced checks and more. You're eligible for a free copy of your report once every 12 months. Request your record in one of these ways: Call ChexSystems at 800-428-9623.
You may obtain your free background report for other reasons as well. The most common reason you may request your free report is if an adverse action has been taken against you in the last 60 days. For example, if you have been denied a job or refused an apartment rental application.
A consumer may call a toll-free number 833-479-2272 to request personal information as described in the CCPA. The consumer may also fill out an online form.
