- U.S. and Canadian Medical School Graduates - Complete 12 months of ACGME training
- International Medical School Graduates - Complete 24 months of ACGME training
- Examinations - Provide proof of passing USMLE Step 3 or COMLEX Level 3.
- Additional Training - Show completion of 36 months of ACGME training at the time of the first license renewal.
Notice: D.O. interns must wait until after officially completing 12 months of training on June 30th to submit their license application. The OMBC will not accept applications submitted before this date.

OMBC: Accepted if sent directly by the medical school to [email protected].
Is a SSN or ITIN required to apply for a PTL?
Not required to start but mandatory before issuance.

Is the Postgraduate Training License renewable?
No, the PTL issued on or after January 1, 2024, is valid for up to 36 months and is not renewable.
Are there Continuing Medical Education (CME) requirements while holding a PTL?
No, there are no CME requirements while you are in an accredited postgraduate training program on a PTL. CME requirements begin after you obtain a Physician and Surgeon license.
Can PTL holders get an "X-License" or "Fluoroscopic Permit"?
No, to obtain an "X-License" or "Fluoroscopic Permit," you need a full and unrestricted Physician's and Surgeon's license. For more information, visit the California Department of Public Health website.
What is the fee to transition from a PTL to a Physician's and Surgeon's License?

When should I apply for the Physician's and Surgeon's License if I am training outside of California?
Apply at least six months before you need your license if you don't hold a PTL for California.
- U.S. Graduates: If you completed more than 12 months of training outside California, you need your medical license by the 180th day of training in California.
- International Medical Graduates: If you completed more than 24 months of training outside California, you need your medical license by the 180th day of training in California.
