Canadian Blood Services Donor questionnaire (2024)


The donor questionnaire captures relevant information about you and your donation. It is a confidential questionnaire that helps decide if you are eligible to give blood.

If there is a question you do not understand, or would liketo discuss eligibility based on your responses, contact 1 888 2 DONATE (1-888-236-6283). This could prevent an unnecessary visit to one of our donor centres. Final eligibility will be determined at the time of donation.

Will this questionnaire let me know if I’m eligible to donate today?

This questionnaire will not let you know whether or not you are eligible to donate today. It is provided here for information purposes only.

If you answer ‘yes’ to any of the questions*, you need to clarify your eligibility to donate before coming in. Consult our ABCs of Eligibilityor contact us at 1 888 2 DONATE (1-888-236-6283) or live chat. 

Final eligibility will still be determined by staff at the donor centre once an official donor questionnaire has been completed.

Am I eligible?

Canadian Blood Services’ confidential donor questionnaire captures important and relevant information about you and your donation and helps determine if you are eligible to donate blood, plasma and/or platelets. 

This is a mandatory questionnaire that requires donors to provide accurate answers and includes questions that are sensitive and/or technical in nature (e.g. questions related to your health, social and travel history). To keep donors and recipients safe, all potential donors need to be able to fully understand the set of questions, respond accurately and be able to communicate fluently with donor centre staff throughout the screening and donation process.

Our donor questionnaire is currently available in English and French – however, we do operate in French at select donor centres across the country (please contact your local donor centre to inquire).

Due to the vast number of languages spoken within Canada, we unfortunately cannot provide interpreters at this time, unless we are booking appointments for a large group of donors (minimum 20 people).  Also, due to the confidential and sensitive nature of the donor questionnaire and screening process, we unfortunately cannot permit donors to bring friends or family members to a donation appointment to communicate on their behalf or to translate screening questions for them.

The following questions ask about medications and shots you may have taken.
  1. In the last 3 days have you taken medication, including Aspirin?(vitamins and birth control are excluded)
  2. In the last month have you taken Accutane, Epuris, Clarus, Absorica LD or Isotretinoin?
  3. In the last month have you taken Toctino, Hanzema or alitretinoin?
  4. In the last month have you taken Proscar, Propecia or finasteride?
  5. In the last 3 months have you had a vaccination?
  6. In the last 4 months have you taken any medication by mouth to prevent an HIV infection such as pre-exposure prophylaxis (PrEP) or post-exposure prophylaxis (PEP)?
  7. In the last 2 years have you received an injection or shot to prevent an HIV infection, such as pre-exposure prophylaxis (PrEP)?
  8. In the last 6 months have you taken Avodart, Jalyn or dutasteride?
  9. In the last 12 months have you had a rabies shot or a shot for exposure to hepatitis B?
  10. Before 1986, did you receive human pituitary growth hormone?
  11. Have you ever taken Soriatane (acitretin) or Tegison (etretinate)?

General Medical

The following questions are about your medical history
  1. Do you have diabetes requiring insulin?
  2. In the last 3 days have you had dental work?
  3. In the last 10 days have you had COVID-19?
  4. In the last 6 months have you consulted a doctor for a health problem, had surgery or medical treatment?
  5. In the last 6 months have you been pregnant?
  6. In the last 6 months have you had hepatitis?
  7. In the last 6 months have you received blood or blood products?
  8. In the last 12 months have you had a graft?
  9. In the last 12 months have you had close contact with a person who has had hepatitis?
  10. Have you ever had malaria?
  11. Have you ever been pregnant, miscarried or had an abortion? (platelet donors)
  12. Have you ever had a positive test for the HIV/AIDS virus?
  13. Have you ever had epilepsy, fainting or been in a coma?
  14. Have you ever had a heart problem or stroke?
  15. Have you ever had a blood or lung problem?
  16. Have you ever had cancer?
  17. Have you ever had Crohn’s disease?
  18. Have you ever received a dura mater (brain covering) graft?
  19. Have you ever had Chagas' disease, Babesiosis or Leishmaniasis?
  20. Do you have a biological (blood relative) parent, child or sibling diagnosed with Creutzfeldt-Jakob Disease (CJD)?

Travel

The following questions are about your travel history
  1. In the last 3 months, did you return from a stay of less than 6 months outside Canada and the U.S.?
  2. In the last 8 weeks have you travelled outside Canada?
  3. In the last three and a half years have you spent more than 6 months in a continuous period outside Canada or the U.S.
  4. Have you spent a total of 6 months or more in a continuous period in Mexico, Central America or South America?
  5. Were you born in Mexico, Central America or South America?
  6. Was your mother or maternal grandmother born in Mexico, Central America or South America?

Additional questions

  1. In the last 3 months have you had a tattoo?
  2. In the last 3 months have you had skin or ear piercing?
  3. In the last 3 months have you had acupuncture or electrolysis?
  4. In the last 3 months, have you had a new sexual partner?
    If ‘yes’ – In the last 3 months did you have anal sex?
  5. In the last 3 months, have you had more than one sexual partner?
    If ‘yes’ – In the last 3 months did you have anal sex?
  6. In the last 6 months have you had an injury from a needle or come in contact with someone else's blood?
  7. In the last 6 months have you taken illegal steroids with a needle?
  8. In the last 12 months have you had or been treated for syphilis or gonorrhea?
  9. In the last 12 months have you been in jail or prison?
  10. In the last 12 months have you had sex with a sex trade worker or anyone else who has taken money or drugs for sex?
  11. In the last 12 months have you had sex with anyone who has ever taken illegal drugs with a needle?
  12. In the last 12 months, have you had sex with anyone who has HIV/AIDS or has tested positive for HIV/AIDS virus?
  13. In the last 12 months have you taken money or drugs for sex?
  14. Have you ever taken illegal drugs with a needle even one time?
  15. Have you, in your past or present job, taken care of or handled monkeys or their body fluids?

PrEP is when people at very high risk for HIV take HIV medication to lower their chances of getting infected. PEP means taking HIV medication after being potentially exposed to HIV to prevent becoming infected.
