The End of Life Option Act(EOLA) allows an adult diagnosed with a terminal disease, who meets certain qualifications, to request the aid-in-dying drugs from their attending physician.The Act requires physicians to submit specified forms and information to the California Department of Public Health (CDPH). CDPH will collect data from forms submitted by physicians. Annual reports will be made pursuant to the Act and relevant privacy requirements.

To enhance user experience and improve accessibility, a new EOLA Online Forms Portal is in development and is expected to launch in late summer.

EffectiveJanuary 1, 2022, the required waiting period between the first and second oralrequest is 48 hours and the Final Attestation for Aid-in-Dying Drug form is nolonger required. Additionally, the text of the Interpreter’s Declaration andPatient's Request for Aid-in-Dying Drug forms has been updated. Please refer toSenate Bill (SB) 380 to review additional changes to the EOLA.

To access the EOLA portal and complete the EOLA forms online please follow the instructions below:
- Click on the "EOLA Online Forms Portal" image above
- Select "Create an Account"
- Select the tile labeled "Center for Health Statistics and Informatics"
- Select "Get Started"
- You may select the form that you want to complete by selecting "Apply." The first form that is required to be entered into the EOLA Portal is the Attending Physician Checklist & Compliance Form.
